What to bring to your first appointment
- Your completed new patient packet
- Driver’s license
- Insurance card(s)
- List of current prescriptions including dose and frequency
- Recent records and test results relevant to the reason for your visit.
- Your medical and surgical history
NCA is a scent free office. Many people are extremely sensitive to perfumes, colognes, and other scented products. For the comfort and health of others please refrain from wearing scented products in this office. Thank you. We believe open communication is the key to any successful relationship.
